2. Please print this form and send application along with a $65 application fee to one of the below addresses. The check should be made payable to "Ohr Torah Stone Institutions" and earmarked "Yeshivat Torat Yosef Hamivtar".
3. Please mail or e-mail (to address below) 2 recent passport photos.
4. Please e-mail the names of two people we can contact for reference. At least one should be a Judaic studies teacher or rabbinic leader.
